I think that this is a manufacturing problem and not a design issue as this type of stem with one bolt is quite common. However I would like to prove this with maths. Any help greatly appreciated. no image . is this on a custom bike of some kind ?, is it possible the clamp does not pinch up tight enough to get a good clamp on the bars ?, you might need to file/hacksaw the gap in the clamp a bit to improve the clamping . if the gap is already big enough then you might just need to roughen the bars where they meet the clamp .
